Suffolk County Council, Suffolk Supporting People and Great Yarmouth and Suffolk PCT’s are committed to a Moving On strategy for people with learning disabilities.
This is a great opportunity for people currently living in NHS Hospital accommodation and in Registered Care Homes to have the opportunity to live independent and fulfilling lives, with care and support as needed, in their own homes.
