Outcome Monitoring was introduced by Communities and Local Government in June 2007. This currently only applies to provider's of short term services, but will be introduced for long term services from Summer 2007.
Outcome forms should be completed electronically and there are three ways of doing this:

Online Web entry system - This method is accessible via www.spclientrecord.org.uk under 'Outcomes'. In order to send St Andrews Outcome information, you require a log-in and password. If you do not already have these, please go to www.spclientrecord.org.uk click on the 'CROSS' section, and 'sign up for web entry'. If you currently submit Client Record Forms by this method, the same log-in and password should also be used for your Outcomes form.

Electron Submission using SP Digital - The SP digital software is available to download from www.spclientrecord.org.uk in the section 'SP Digital' 2007/8. This method allows you to complete your forms on screen.

The form will be available as a word document from www.spclientrecord.org.uk in the section 'SP Digital 2007/8'. This method allows you to submit Outcome forms using your own system. Data should be emailed as an attachment to outcomedata@st-andrews.ac.uk.
